New Care Management Model
Transforming Patients' Lives

As a partner in the Bangor Beacon Community St. Joseph Healthcare has been able to develop a very successful model for care management of people struggling with heart conditions, chronic lung disease, asthma and diabetes. Jessica Taylor, Lead care manager under the Beacon grant for St. Joseph Healthcare, has had incredible success at improving the lives of participating patients through personal one-to-one coaching and medical guidance. Here's the story of one local couple who have greatly benefited from Jessica's efforts.

Patients actively participating in a care management program, both locally and nationally, show reduced hospital stays, reduced emergency department visits and improved overall health status. Based on these very successful results, St. Joseph Healthcare is adjusting the current Cardiac Rehabilitation programs to a Cardiac Care Management Program. Story
